Try this on the 3576 Incoder first, if it works you can bring it over to the 3569 Incoder as well, however, 
it may be preferable to upgrade the whole system to 3576 (as it has stability fixes I’d like you to do to air with). 
-	Shut down Incoder (Start > All Programs > XPression Incoder > Stop Incoder)
-	Replace C:\Program Files (x86)\XPression INcoder\bin\xpVideoSourceQT.xvs with this one:

-	Restart Incoder
